Found the problem. I was saving an object reference to the toolbar in a custom property of the form; i.e., ThisForm.oSavToolbar = ThisForm.oToolBar. After I commented the line, everything worked perfectly.

However a question remains: Why would saving an object reference to a separate form property cause the toolbar to be displayed on the ListObjEdit form?
