When you reply, the "To" person is the person who's message you clicked "Reply" from, but you can send copies to other people by clicking the check boxes by their names at the bottom of the edit window. Some people may have participated in the thread but their names may not appear because they have configured their accounts that way, eliminating a lot of extra stuff in their Replies box (if they want to read it they'll follow the thread). The name in bold is the name of the person who originated the thread.
