Your COM server is blowing up, for whatever reason. Is this happening when the object is instantiated, or when you call certain methods? If the former, what's in the Init code that could be suspect? Same question if a method is causing it to crash. Have you installed SP4, and installed the SP4 runtime libraries on the server, as well as the recompiled app?
