>I followed the instructions. I indeed refer to what happens when I copy the 32x32 icon and then select Edit - New Image. Since I see a fox in Explorer, and since I don't remember ever making the 16x16 version, I conclude that my .ico does not have the 16x16 version in it. Furthermore, if I just create a new icon, the 16x16 option isn't there either.

What version of the Image Editor are you using?
