I resolved problem this way, see if this can help you

1. Driver: GENERIC
PAGE BREAK

2. PageSize : CUSTOM

3. You need to adjust page length IN PRINTER by sending escape sequence, or
mannualy to follow CUSTOM size (as in 2.)

4. You have to clean up frx file before compiling
(settings in first record)

5. Prior to printing you issue:
set printer to name (UserPrinterWindowsName)
