Simply setting the "No" button to be the default will require consicious action on the user's part to either press "Y" or click it with the mouse. If they then do so, it becomes their responsibility.
This is probably a good place to recommend a great book I just read. The title is "GUI Bloopers: Don'ts and Do's for Software Developers and Web Designers". Author is Jeff Johnson. Publisher is Morgan Kaufmann.
I don't think there's any question that this book (and pretty every other interface book I've ever read) considers the sort of confirmation dialog we're talking about here useless. The users learn the right keystrokes and use them without thought. The best solution is to make the action reversible.
