We have our local email, and everyone has their internet email too, with the same account. We use something called Kinesphere as an internet gateway, so when someone gets an internet email, its sent to their MS Mail account.

I'm using Deerfield's MDaemon as an alternative to Exchange 2K. I'm quite happy with it for our use at eSolutions Services, but in essense, all our mail is routed via the Internet, with no central office to speak of.
