You really haven't given us much to go on.

What were the nature of the "changes, revisions, etc."? For instance, did you change buffering technique?

I guess it could also be a combination of the application's design and a change in usage pattern with the users. For instance, maybe they have started looking at each other's active information to help each other out. If certain information is visible only in "Edit" mode, then that might explain things.
I guess they might even be opening two windows on the same system to the same record in edit mode.

I think you'll need to get, and provide here, some more detail.
