You may want to look up Opportunistic File Locking on the NT server. We had nightmares until I figured out that we needed to disable this. Since I disabled that option we have not had another problem with duplicate numbers or RLOCK's not getting released.

There is a kb article on msdn about this. Just a note too.. we have not noticed any performace penalty or side effects of setting EnableOPLocks to 0.
