Typically, some component required by the OCX is not present - a missing DLL, or the wrong version.

The registry might be full. THe maximum size is tied to the size of available virtual memory.

Under another OS like WIn2K or WinNT, the registry segment might be protected.

The drive containing the registry may be full, so it can't be expanded.
