You must include images for both 16x16 and 32x32 resolutions in the .ICO file - Windows will display both image sizes at different times (large icon vs small icon; in Explorer,switching the View menu choice between Large Icons and Small Icons will switch between the icons, and you can tell Windows which icon size to use through the Display control panel applet (on this machine, it's controlled by a checkbox labelled "Use Large Icons" on the "Effects" page tab.)

If your icon contains only one of the necessary images, Windows will use the VFP default icon (based on the default string registry value under the DefaultIcon subtree of the named file type entry within the hive HKEY_CLASSES_ROOT of the registry) when it needs to use the missing image.
