We also tried using project properties to specify the Icon file. We tried both 16 and 16 + 32 files with no success: the Icon correctly appears in the top left of the app, but the Foxhead always shows in Windows Explorer.

It sounds like you're doing everything correctly

Out of interest... if you try changing your view settings in windows explorer (large icons/small icons) does the icon appear at all?
