As the author of the FAQ, it doesn't sound like you're missing anything. However, it could be that the order the icons appear in the file may have something to do with it. IOW, you may have to start with a 32x32 (16 color) icon and add the 16x16 to it, rather than the other way around (a SWAG, BTW to be sure).

I don't know if this additional information will help, but I've found that if you have a 32x32 256 color icon, and a 16x16 16 color icon in the file, the Fox shows for the main icon, but the 16x16 shows in the explorer.
