The military has been playing around with "duplex" rounds for many years, most recently in the AIWS (Advanced Infantry Weapons System) experiments in the early '90's. As far as I know there was never any wide-scale testing, up to that point, of duplex rounds in the M16, although there were several versions field-tested in 7.62x51 (.308) during Viet Nam. Duplex cartridges have never really reached production, partly because of cost, partly because of the unpredictable impact point of the second bullet.

The M16 does tend to cause large wounding in the body relative to the size of the bullet, in part because the bullet rapidly looses stability on impact. In flight, however, it is as stable as any other bullet fired from a rifled weapon.
