Yes, I hate that too, and it happens in far too many indexes these days.
As I had said, it was more particularly the .CHM file that I was interested in, but the index is important too.
Looking at "The Fundamentals" and "Internet Applications" books (as examples) the vast majority of entries have only 1 page number referenced, with possibly 2% having 2. Things are mentioned far more often that that (in virtually any programming language) in these books and restricting entries to a single/double reference removes much value.
Of course the fact that these books exist at all far outweighs these problems, but improvements in these areas would certainly add significant value to them.
