First to get the icon, you have to add it to the project from the Project Info selection on the Project menu (Project tab of the dialog). In order for it to show up as the icon in the Windows Explorer, the icon file must have both 32x32 and 16x16 icons in it. In order to get an item in the Start Menu's Programs, you have to indicate at least one file as being a PM Item, by selecting that check box in step 6.
