I'm not sure there is a standard for some of these, but here's what I typically use...some, like phone, SSN and zip are standard.

Phone- 10 (no formatting saved)
Addres- 40
City- 30
Social Security No.- 9 (no formatting saved)
Zip - 9 (no formatting saved)
First Name- 20
Last Name- 20
Second Name- 20
