It's probably mostly related to inexperience at the time, but I've found our dynamic menu's hard to keep track of. For certain places in our system the menu can change based on several different variables (user, security, currently selected record, prefereces, etc.). It just got darn complicated! When we redesign our product from scratch (Which i'm told is a month away, Woohoo!) rest assured I'm stomping on that bad boy right off.

It works (I think, as I said it was our first try so we were inexperienced) but plan it well in advance, especially if it's going to be complicated.
