Thanks for your reply. The toolbar control is the Microsoft Toolbar Control 6.0 (SP4).

I finally solved the problem yesterday, the form was just hidden, don't ask me why. Adding the command this.visible = .t. in the init() seems to sort the problem. Why the form would be hidden just by adding a toolbar I don't know, but like you say ActiveX controls seem to behave a bit weird in VFP.
