When you are running your form, ALT+PrintScreen to capture the form to the clipboard, then paste into Word. I would recommend pasting into a photo editor of some kind so that you can save the picture as a .gif or .jpg though. If you paste directly into Word, it'll be a .bmp which will quickly make the document huge (> 1MB).
