I used to do that (Cobb CEE) and probably still should but now with source safe it's easy to see the differences between versions so it's not as critical to me. I will still do it for any red flag sort code changes to cover my butt: e.g. "per John Jacobs request 2/21/01 requisitions from the USS Amigo..." this stuff should be documented in whatever system one uses to track user requested changes but having it in the source code can make it easier to find. And it makes me feel more secure.
