There were blocking issues with the initial release of VFP6 that would cause what you are seeing. These were fixed in SP3.

When you select "Build" from the project manager, you need to select "Multi-threaded COM server (dll)".

When you create a setup with the Setup Wizard, if you check to include them, they will get built into the setup. I wanted to make sure you weren't compiling with SP5, but running on pre-SP3 runtimes.
