I'm getting the message, "Error instantiating the object".

Here's the code:

ox = CREATEOBJECT("PatientMaintForm", "TDBM")

I get no other errors. I've recompiled everything and get no errors. There are no ActiveX controls or COM Servers associated with this class. Any ideas what is causing this error?
