This has changed a lot over the last several years. ProCD used to allow you unlimited export to a table from their proprietarily compressed data format. However I think ABI (?), the new owners, disabled this feature for fear of the potential to set up competing phone matching services. I haven't looked in several years, but I believe it will still be the case that the available phone books on CD-ROM are all stored in proprietary data formats and designed to prevent direct access.
The people who maintain nation-wide phone databases do so on mainframes. You're talking 100GBs, if not terabytes of data if stored in VFP tables. And keeping the data up-to-date is a nightmare.
I'm not sure what you're trying to do, but I know some people who have had success with programs that make requests to the various phone web directories.
