Not sure it this is your problem, but this has happened to me when I had a bad relationship or SQL result based on two tables.

Instead of displaying each of the records that appear in the table, it displays mutiple copies of the first record. There's no filter and deleted is off. The number of duplicating records doesn't match the reccount() for the table either (there are fewer in the report). The cause might be right under my nose but I can't figure it out.
