Did your "OpenAlias" property in DataEnv is filled with the database/table? The error message was not caused by your form file, it came from the database/table. You can try to remove the statement of database/table. BUT, the form without database/table is meaningless. When you put back the statement, you may find out another error. Therefore, please do not use your useful files to do the test.
