For both, we're using Wise InstallBuilder. It's especially good for taking care of the smaller updates;"SmartPatch," I think, is Wise's term.

As far as the DBCs, we use separate ones for each client and a "common" one for most MM tables. We're up to 7 distinct dbcs, and soon to go over 10. Stonefield is invaluable for keeping them in sync!
