Verify if you have a table in the dataenvironment of your form
In the Form Editor
Right click on your form, then select Dataenvironment
If a dialog appears for select a table then select your passwords table and click Ok then Cancel
If apears the dataenvironment window with your password table then click on the Properties window and select the datanevironment
Look for the InitialSelectedAlias and select your table

Check on your form if in some one place your table is closed or there are a USE command without a table name
