I went through this situation. There were no miracles or bugs.
Here are the reasons I met:
1. In the main program I call report.prg instead of report.app.
2. Old version of report.app was in the path
3. I call frx indirectly, mean from report.app I call another app that call frx.
4. frx was included in the project but marked as excluded.
5. old compiled version of report.prg (report.fxp) was in the path
