Assuming the network is not flooded by other talk, a 10-user fp app for a medical/dental practice should be quite quick. I'd say the immediate "culprits" to look for are:

1) locates or views against big tables using unindexed expressions;
2) table file header locks (caused by replace for / append blank which "lock" the whole table; use update / insert instead to get record-level locking, or move to views)

If it isn't one of those, let me know.
