the reality is that users and customers see problems as "software problems" even if caused by their own stupidity. Much easier to add a simple facility that highlights the problem so the customer appreciates that they must either pay for the fix every time their staff cause it, or prevent it. Makes for an easy life- no matter how much the client pays, regular disruption for "urgent" fixes in a different town is a stress even for large teams.
