You may want to have a look at www.stonefield.com. Stonefield Database Toolkit will do this and much more.

With SDT all you have to do is send your new Exe and a set of Metadata files to the clients site and SDT's Update Function will handle the rest. It handles creating new tables, adding, renaming or removing fields as well as updating or changing CDX's. Any structural change you have made to your development free tables will be duplicated at the clients site. With the set of metadata that SDT uses update your data structures it can also use that same set of metadata to handle fixing any header problems with DBF, CDX and FPT files.
