I dealt with a similar issue, while I can't offer any other advice on how to recover your form I can offer you some advice on how to stop it from happening in the future.. There is a known VFP/NT error with disk write caching. If your computer is shutdown prematurly(ie. crashes) or sometime the write buffer is not cleared on normal exit. This was confirmed with Microsoft and there recomendation is to turn off write caching
