We use Web Connection for both internal use and for client development work. We have a dedicated 56K Frame Relay connection that costs about $375/month for UUNet and $135/month for Bell Atlantic. I'm sure more and possibly cheaper options are available, but this has been very reliable for us.
Start-up costs were another $3,000 or so (not counting file/web server box, which we were using anyway). This was primarily for router and CSU/DSU units (which are now cheaper and available as a single box).
