Has anyone had any experience getting the following Visual AccountMate error?

Error(1520): No database is open or set as the current database.

We have a slightly modified Visual Accountmate Inventory Maintenance
(icmitem) [LAN Version build 3.09 VFP 5.1] form that can launch a custom form to perform a non-inventory function. The custom form uses a shared data session
and references tables in the calling form. If (1) the custom form is launched and then closed and (2) icmitem is then closed and (3) we then open another form [i.e. Customer Maintenance], we receive the error above. The error continue until we exit VAM and come back in. We have reset the AMW database in the unload event of icmitem and is it is open and is the current database. We have made hundreds of Accountmate Modifications and have never seen this error before and cannot trace it. [Accounmate regretably does not provide full source code]. Any ideas?
