Does anyone know the specifics regarding the information that must be supplied to MS in order to get the rebate?

Step 2 on my rebate form indicates I need to send in my product box top. I'm a little nervous about sending in my actual box top (vs. a copy). What if my rebate request gets lost and I need to re-send it? Likewise for the title page from my VFP 6 user manual.

If anyone from MS is monitoring this thread, a MS specific opinion would be appreciated. Of course all other opinions are welcome, too!
