We have also seen these errors, randomly, on less than 1% of our users.
We always find the problem to be related to caching the writes.
On a single-user, virus scanners, power savers, and screen savers sometimes interfere with shared files being written back to the hard drive.
In a multi-user configuration, Win2000 automatically turns on caching. NT also has a caching feature that needs to be turned off.
Some hardware caches the writes. One tech told me he finds IPX protocol to create problems with network traffic.
Even if your code forces the write, the OS and hardware will control the true file state and accessibility.
