Well it turns out that the rebate is:

"This offer allows one rebate of U.S.$50/CDN$70 per coupon".

Just thought you'd want to know.

Other irregularities:

MS web site says Professional or Enterprise versions of VS 97 or VS6.0 qualify, but coupon does not.

MS web site says offer expires July 31, 2002 and coupon says December 31, 2002.

I guess we should just get used to sloppiness.
