I've had this a couple of times, and after some debugging, it turned out that some code in the form's .init was, once it was done, sitting on an alias which was at eof(). For some reason, that hosed the grid, which was based on an entirely different alias. There was a scan-endscan block and a simple Locate at the end of the form.init fixed it.
