I want to buy a book from Hentzenwerkes and use my PUTM discount, of course.

When I did this the first time there was a special order form to be downloaded.

This time I have not been successful in locating any similar form, nor in finding some way to specify PUTM in the standard order form.

Can someone tell me the procedure to use now?
