I have seen it in a particular FPW2.6 application in 32 bit windows - Win 95 and above. It always happened in the same place. Sometimes it would run fine, then the next -poof- I was looking at the Windows desktop. The application just disappeared. No error message, nothing. Just gone. I rewrote that section several times and was never able to fix it, and finally had to remove it from the app.

Wish I could give you a solution.
