See thread #552662. Basically your icon file must contain both 32x32 and 16x16 icons.

>I created a project and initially I did not assign any icon to the project. When I created the executable it had the Fox head as icon. Later I went to Project properties and assign a proper Icon for that application. I recreated the executable, but it still has the Fox Head as Icon.
