You could probably do it with the OpenDwg Toolkit.
http://www.opendwg.org/index.htm. I just looked at they have an activex control for VB. They've reverse-engineered the .dwg file.
FYI, OpenDwg is NOT supported by AutoDesk and AutoDesk frequently changes the format of the .dwg file.
I don't think there is any other way to do it, but you could ask on Autodesk customization newsgroup.
