Medical Manager seems the most established and widely used one. Our system (look at www.clinicsoftware.com) is a clinic management system but we prefer to integrate with existing billing packages, rather than reinventing the wheel. It does have demographics, scheduling, etc. It does do medical history and other medical procedures too, so if they're also interested in that...
