One thing I did to teach myself about it was to download the trial version of ForeHelp, build a few things, and then reopen the things using HTML Help WorkShop, Microsoft's free, poorly documented basic HTML Help builder. It helped me to understand context sensitive help. Downloading and trying the West Wind builder would also be instructive, especially to see the different way the context sensitive help file gets created.
I did this stuff so long ago that I don't remember it all so well now.
