What are the permissions for the directory where the COM object is located? Most likely, the IUSR_ doesn't have rights to it.
Another approach on Win2K is to configure the COM object as a COM+ application. With the COM+ Explorer, you can make the COM object instantiate under the security context of a user that access rights. You can get a full description of how to do this by reading Craig B. articles in Foxtalk starting a few months back.
