1. There could be several reasons why it is happening, so it's difficult to say exactly why you are seeing this. There is a document on my web site that lists several possible causes.

2. Can you post your PK generation code?

Records are disappearing from files. 2. Primary Keys are being violated - that is, duplicate key fields are appearing in tables and the primary key index is being dropped.

I do not know if this is going to fix the problem.
