After further investigation, I believe that the headers are not causing the problem at all. I commented out the code that changes the headers and I still got the error, just with a different name. I now believe that it has something to do with the parameters being passed to the form I am calling. When I eliminate the last parameter, it works! Now I am trying to figure out why this is so.
