I believe this is by design (the summary printing on a separate page--not so sure about the missing header problem).

To get around this, create a data group on eof(), and put your summary objects into the group footer.

If you do find you want to use the summary band on a separate page, and you want the header and footer info, what I do is copy all the header and footer objects into the summary band. You'll have to make the height of the summary band the same as (or slightly less than) your page length.
