I have User's form and couple of classes designed for this purpose. I used multiselect grid (my modification) for Towns criteria. Range class for zipcode range and textbox+list class for typing individual zipcodes (not in range). The first (draft) implementation of my classes could be found here in Download section (Input classes and Container based on Multiselect Grid class). You may take them as a start point. I can probably send you my whole class library with all classes implemented (Range class, checkbox class, Input class, MultiList and MultiGrid), if the aforementioned classes would not be enough...
